Network Access and Termination Policy
Purpose: The purpose of this policy is to implement procedures for establishing and terminating access to the UA network and business systems supported by Financial Affairs Information Systems Support.
Scope: This policy applies to all Financial Affairs network users. Any exceptions to this policy must be approved by the AVP for Administration.
Policy: Ensuring appropriate access to the UA network and Financial Affairs (FA) business systems is a critical part of maintaining a secure technology infrastructure. The following procedures should be adhered to when adding/terminating FA employees.
New Employees Supervisors are responsible for promptly notifying FA IT when new employees are added to their workforce. IT is responsible for setting up network access, FA Email access, and appropriate FA business systems access.

Terminated Employees (and other Employees to Whom Access Should be Denied)
Supervisors (or other responsible persons) are also responsible for promptly notifying FA IT when employees are terminated (voluntary/involuntary) from their workforce, or when access should otherwise be denied. IT is responsible for disabling network access, FA Email access, and appropriate FA business systems access for the terminated employee.
This form should be used to request denial of access for employees.
A report listing all terminated FA employees will be reviewed weekly by FA IT. If an employee is listed on the report in which a supervisor request for account termination has not been submitted, FA IT will notify the appropriate supervisor (or other responsible persons) via email that action needs to be taken. The supervisor will be given one business day to respond to the email. If no response is received by FA IT, it will be assumed that FA network access, FA email access, and FA business system access should be disabled.
